On January 4, 2025, the Guangdong–Hong Kong–Macao Greater Bay Area CHIVA Academic Salon on varicose vein treatment was held at the Shenzhen CHIVA Center of Dr. Smile Medical Group. Vascular surgeons from Hong Kong and mainland China gathered in Shenzhen to exchange ideas on the hemodynamic concept and its clinical application in the treatment of venous diseases.

The meeting was chaired by Dr. Qiang Zhang, founder of Dr. Smile Medical Group. In his opening remarks, Dr. Zhang noted that as hemodynamic concepts continue to gain attention in the management of chronic venous insufficiency, more physicians are beginning to focus on preserving venous function and reconstructing physiological venous drainage pathways. Academic exchanges across regions, he said, play an important role in deepening understanding and promoting clinical application of these concepts.

The salon featured the participation of a consultant vascular surgeon from Queen Elizabeth Hospital Hong Kong, as well as physicians from several cities in mainland China. Participants engaged in discussions on the diagnosis and evaluation of chronic venous insufficiency, hemodynamic analysis, and clinical experiences related to CHIVA (Conservative and Hemodynamic treatment of Venous Insufficiency).

During the event, Dr. Deng Jianping of the Shenzhen CHIVA Center conducted a live case demonstration. Through real-time ultrasound evaluation and procedural explanation, he illustrated the role of hemodynamic assessment in preoperative decision-making within the CHIVA approach. The demonstration prompted active discussion among participants on identifying reflux pathways, determining shunt points, and designing individualized treatment strategies.

CHIVA is a vein-preserving treatment strategy based on hemodynamic assessment. Unlike traditional approaches that focus on vein removal or ablation, CHIVA emphasizes analyzing venous reflux patterns and reconstructing physiological drainage pathways, thereby improving venous function while preserving the venous structure. In recent years, this concept has attracted growing interest in the international field of venous disease management.

As one of the most dynamic regions for medical collaboration, the Guangdong–Hong Kong–Macao Greater Bay Area provides an important platform for cross-regional academic exchange. The salon not only facilitated professional interaction between physicians from Hong Kong and mainland China, but also created opportunities to further promote hemodynamic concepts in the treatment of venous diseases.

Dr. Roberto Delfrate reviewed 23 years of clinical practice and shared the experience of CHIVA in Italy.

Dr. Smile Medical Group stated that it will continue to support academic exchanges and training programs related to the CHIVA concept through case discussions, clinical demonstrations, and educational events, contributing to the ongoing development of venous disease management.

The academic salon concluded successfully after extensive discussions. Participants agreed that such clinically focused exchanges are valuable in advancing the understanding and application of hemodynamic principles in venous disease treatment.

Dr. Qiang Zhang
Dr. Qiang Zhang is a vascular surgeon with more than three decades of clinical experience in the treatment of venous disease. His work focuses on the hemodynamic understanding of varicose veins and the development of vein-preserving treatment strategies, including the CHIVA method. Over the course of his career, Dr. Zhang and his team have treated more than 100,000 patients with varicose veins, contributing extensive clinical experience to the field of venous medicine. Dr. Zhang is the founder of Dr. Smile Medical Group, a network of vein centers dedicated to the treatment of chronic venous disease. Through clinical practice and physician education, the organization promotes approaches that aim to preserve the physiological function of the venous system while addressing venous insufficiency. He is also the initiator of the Global CHIVA Center Program, an international initiative that supports physician training, clinical collaboration, and the development of CHIVA-based vein centers. Dr. Zhang serves as Executive Chairman of the Asian Venous Academy, promoting academic exchange and professional education in venous medicine across Asia. His work is guided by a fundamental principle: the treatment of varicose veins should respect venous hemodynamics and preserve the natural function of the venous system. Rather than simply eliminating diseased veins, he advocates approaches that restore physiological circulation and maintain the integrity of the venous network whenever possible.
